Of the ten conclusions, keep these five first

Pharma’s ceiling is set by payers, not by science

What is still unclear?

Write what you still want explained. Questions go to the author and are not shown publicly.

  1. The profit pool is in the US

Global drug market ~$1.7 tn. Tiny population, huge spend—profit follows the payer.

US drug-price shocks move the whole industry’s valuation center.

  2. US pricing is being rewritten

The US premium was put on the negotiating table as a system.

Not one headline—the profit base is being rewritten.

  3. A super winner is a five-factor product

Need × edge × patents × own commercial × deep focus

The right category is not enough; one zero zeros the formula.

  4. Going overseas is arithmetic

The same molecule meets a much larger payer.

A domestic peak is often a fraction of US value.

  5. Discount the headline total

2026H1 out-licensing: big headline, small cash.

Give hard value only to upfront and already-hit milestones.

These five lines are the reading lens for the whole book. Later chapters expand one judgment each. The numbers are anchors, not full statements.
